Overview

As our Assertive Community Treatment (ACT) Registered Nurse, you will be providing case management services to the people we serve within the ACT program. Daily visits, medication pick up/set up/drop off, assisting with injectable medication, skills building, along with other needs while in the community are among the things you may find yourself doing. You will also manage prescriptions from the doctor, monitor lab work, and manage requests for physician actions. This team-oriented program offers training to anyone willing to learn new things.

Hours – Full-time hours are Monday-Friday, 8am-5pm, with a rotating on-call once trained.

Must have – Your Registered Nurse license in the state of Michigan.

Salary - $25.61-$35.37/hr

 

What is Assertive Community Treatment?

Assertive Community Treatment (ACT) is an evidenced-based model with a therapeutic set of intensive clinical, medical, and psychosocial services provided by a mobile multi-disciplinary team. Services are individually tailored and include case management, psychiatric services, counseling/psychotherapy, housing support, substance use disorder treatment, and employment and rehabilitative services. Services are provided in an individual’s home or within the community and include availability of multiple daily contacts with 24/7 crisis availability.

ACT is provided to individuals with a primary diagnosis of serious mental illness (SMI) and who, without ACT, would require more restrictive services and/or settings.
