About This Service
Integrated Health Care is a care coordination system of general and behavioral healthcare, integrating mental health, substance use disorders, and primary care services. Individuals receiving services with at least one of five chronic, comorbid conditions (COPD/asthma, cardiac, obesity, diabetes, and high blood pressure) are identified. Staff coordinate services with the individual’s primary care provider, and/or specialists, to produce the best outcomes and provide the most effective approach to caring for individuals with multiple healthcare needs. Regular meetings are held to address care coordination with local Federally Qualified Health Care (FQHCs) to help manage physical healthcare needs of those mutual individuals served.
